Patient-Centered Care

Patient-Centered Care

Even before it became a national standard, a patient-centered approach was our goal for high-quality health care.  This is care tailored to each patient’s unique needs and preferences.  Today the evidence is clear that patient-centered care increases quality, improves health outcomes, and reduces costs.

We recognize every patient has unique needs.  When we tailor services for you, we consider your primary language, generational issues or attitudes, and other factors to create a plan that meets your individual needs.  Our health care professionals take a personal interest in helping you achieve a healthy life in an encouraging and nurturing environment.

“It’s about patient-focused care and a team-based approach to care to ensure patients get the care they need when they want it, how they want it, and in a language and manner they understand.”

What is a Patient-Centered Primary Care Home?

The term ‘health home’ is being used to describe the feeling of knowing who to ask when you have questions about your health, and where to go to have your health needs met.  When you join our Health Center family, you have a team of professionals who care.

Your Patient-Centered Primary Care Home will . . .

  • Better coordinate your care to help get you the services you need, when you need them
  • Listen to your concerns and answer your questions
  • Offer after-hours help and alternatives to the emergency room
  • Help you plan an active role in your health
